The article, by Thomas Begley, Jr. and Emily Schurr of the Begley Law Group in Moorestown, NJ, details how to create a life care plan that can serve as a guide for relatives and caregivers of a disabled individual.
A life care plan details information about the disabled individual and their family, what benefits and resources are available to them, their preferred health care providers, and what to do in case of an emergency. The document can also go into detail about the disabled individual’s preferred living arrangements, what sort of assistance, if any, the disabled individual needs on a regular basis, and their social abilities and preferences.
The plan can also be updated over time as the disabled individual’s circumstances change and can be a valuable resource for the individual’s caregiving team and when creating estate planning documents for the benefit of the disabled individual.
